What GLP-1s cost without insurance: every real path, cheapest first
Skip the ads. In August 2026 there are exactly four legitimate ways to pay cash for a GLP-1, and they sort cleanly by price. Here they all are, with the honest trade-offs.
Step 0: check insurance anyway
Even if you assume you're not covered, spend ten minutes confirming. With commercial insurance that covers weight-loss GLP-1s, a manufacturer savings card takes Wegovy® or Zepbound® to as low as $25/month — one-sixth the cheapest cash option. And since July 1, 2026, eligible Medicare Part D members can get certain GLP-1s (including Foundayo™ and Zepbound®) for $50/month through manufacturer programs. Check your plan formulary, or pick a provider with prior-auth support (Ro, LifeMD, and Found's $49/mo insurance tier all do this legwork). Nobody pays us anything when you use your insurance — it's first because it's cheapest.
The pills changed everything: $149–249/month, FDA-approved
Until recently, cash-pay meant either expensive branded injectables or gray-zone compounded vials. Two FDA-approved oral GLP-1s broke that trade-off:
- Foundayo™ (orforglipron) — from $149/month self-pay via LillyDirect. Lilly's once-daily pill, FDA-approved April 2026. $25/month with commercial-insurance savings card; $50/month for eligible Medicare Part D.
- Wegovy® pill (oral semaglutide) — $149–199/month self-pay via NovoCare® Pharmacy ($149 is a promotional 4 mg price listed through Aug 31, 2026; the standard price is $199).
At these prices, an FDA-approved pill costs less than most compounded injectable programs. For a first GLP-1 on a budget, this is where the analysis should start. You need a prescription — from your own doctor (free beyond the visit) or any telehealth provider.
Manufacturer-direct injectables: $299–449/month, no membership
Both manufacturers sell direct to cash-pay patients. Any licensed prescriber can send your prescription there — you do not need a telehealth membership:
| Medication | Self-pay price | Channel |
|---|---|---|
| Foundayo™ (oral) | from $149/mo | LillyDirect |
| Wegovy® pill | $149–199/mo | NovoCare® Pharmacy |
| Zepbound® vial 2.5 mg | $299/mo | LillyDirect (single-dose vials, cash-only) |
| Wegovy® / Ozempic® injection | $349/mo | NovoCare® (Wegovy HD 7.2 mg $399; Ozempic 2 mg $499) |
| Zepbound® vial 5 mg | $399/mo | LillyDirect |
| Zepbound® vial 7.5–15 mg | $449/mo | LillyDirect — requires refill within 45 days to keep this rate |
Gotchas: Zepbound vials are syringe-drawn (no autoinjector pen) and can never be billed to insurance; the Zepbound maintenance-dose price assumes on-time refills; Novo has run $199 introductory offers on Wegovy/Ozempic that come and go — check NovoCare's current price guide at order time.
Compounded programs: ~$169–299/month, eyes open
Pharmacy-compounded semaglutide and tirzepatide remain the cheapest injectables — Mochi ~$178/mo all-in, SkinnyRx from $199, Found from $169 with a 12-month commitment, MEDVi $299 ongoing. Three things to know before choosing this lane, expanded in our full explainer:
- They are not FDA-approved products — no FDA review of safety, effectiveness, or manufacturing for the specific product you receive.
- The legal pathway (patient-specific 503A prescriptions) is under active regulatory and litigation pressure, and offerings can change mid-treatment. Have a branded plan B.
- Since the pills arrived at $149–199, the compounded discount vs. FDA-approved options has narrowed to roughly $0–50/month for many patients — do that math for your dose before accepting the trade-offs.
Telehealth memberships: what the extra money buys
Ro ($149/mo, ~$74 prepaid annually), Hims & Hers ($149/mo), and LifeMD ($149/mo, ~$75 annually) sell branded medication plus a membership. The medication costs roughly the same as manufacturer-direct — the fee buys async prescribing, dose management, refill logistics, and insurance/prior-auth fights. Worth it if you want a single app handling everything or you're pursuing insurance coverage; not worth it if you have a prescriber and can order direct. That's the honest framing the ads skip.
